Cornerstone supports people living with Alzheimer's disease, dementia, and memory loss. The community is built around familiar routines, personal care, secure surroundings, and family communication.

Care may range from verbal guidance and reassurance to more hands-on support when those needs can be safely supported in a personal care setting.

Cornerstone is a Pennsylvania personal care home with care staff present around the clock and RN leadership.

Support may include

  • Daily personal care and hygiene routines
  • Meals, snacks, and familiar daily rhythms
  • Verbal guidance, memory cueing, and reassurance
  • Mobility and hands-on personal care support
  • Altered diets and mealtime assistance when appropriate
  • Higher-support needs such as transfers, repositioning, diabetic support, and wound-care coordination when appropriate
  • Family guidance through changing stages of memory care
  • Hospice education and coordination when appropriate
  • Family communication by phone or video when needed

Cornerstone operates with a philosophy of aging in place. The goal is for residents to remain in the community as Alzheimer's or dementia progresses cognitively and physically, as long as those needs can be safely supported in a personal care setting.
